आप 1 Family के 2 Himcare Card कैसे बना करते है?

अगर आपके Ration Card में 5 से ज्यादा Family Members Add है तो जब आप 2 Himcare Card के लिए Apply करें तव आप Ration Card No डालते Time Last में Zero लगा दें

Example:- HP-00000-00000000

3 Himcare Card बनाने के लिए 2 Zero लगा ले Simple.

यदि किसी परिवार में 5 से ज्यादा सदस्य हैं तो उनके दो कार्ड बनवाने होते है।

Note:- Ration Card No बिलकुल सही होना चाहिए (0)

अगर आपके माता पिता कि Age 70+ है यानी Senior Citizen Category में आते है

तो आप उनका अलग Card वना सकते है और अपना अलग इसी तरीके से

आयुष्मान कार्ड या हिम केयर कार्ड बनवाने से पहले जरूरी है कि आप अपनी पात्रता की जांच करें, जिसके लिए आपको पहले इसके पोर्टल https://pmjay.gov.in/ या https://www.hpsbys.in/ पर जाना होता है

आयुष्मान कार्ड वो लोग बनवा सकते हैं जिसमे सरकार द्वारा गरीबी रेखा से निचे जीवन यापन करने वाले परिवार व ऐसे परिवार जिनका नाम सामाजिक, आर्थिक व जातीय जनगणना की सूची में जिनका नाम है। ऐसे सभी परिवार आयुष्मान कार्ड के लिए पात्र हो जाते है

और Himcare Card वो भी बनवा सकते है जिनका आयुष्मान कार्ड कि List में Details Add नही है

Following documents are mandatory for enrollment under HIMCARE:

Category	Documents required for authentication
BPL	Copy of BPL certificate attested by the Panchayat Secretary within previous one month.
Registered Street Vendors	Registration Certificate attested by the Executive Officer, MC/NP/NAC within previous one month.
MNREGA Worker	MNREGA Job Card and Online MIS Report indicating 50 days work under MNREGA in previous or current financial year duly attested by concerned Panchayat Secretary/BDO.
Ekal Naaris	Certificate to be issued by Child Development Program Officer (CDPO) of the concerned area and shall include Widows/Divorced/Legally Separated /Unmarried more than 40 years.
Disabled >40%	Medical Disability Certificate showing permanent disability.
Sr. Citizens above 70 years of age	Any valid age proof
Anganwari Workers/Helpers	Certificate from Child Development Program Officer (CDPO) of the concerned area.
ASHA workers	Certificate from Block Medical Officer (BMO) of the concerned area.
Mid-Day meal workers	Certificate from Block Elementary Education Officer of the concerned area.
Contractual Employees	Certification from concerned Department
Daily Wage Worker	Certification from concerned Department
Part Time Workers	Certification from concerned Department
Outsource Employees	Certification from concerned Department

Himcare Card Apply करते Time अगर Duplicate Aadhaar Card का Option आता है तो उसे कैसे solve करें?

अगर New Himcare Card Apply करते Time Duplicate Aadhaar Card का Option आता है तो हो सकता है उस व्यक्ति का Himcare Card पहले से वना हो

उस व्यक्ति के Aadhaar Card No से आप एक बार Himcare Card Download करके देखे Download कैसे करना है इस Video को देख कर जाने (Click Here)

अगर उस व्यक्ति का नाम Himcare Card मे Show होता है तो आप उस Card को Renew कर सकते है या फिर उस व्यक्ति का नाम इस Card से Delete करने के वाद आप अपने Cart में Add कर सकते है (जब वह अपने पुराने Card से Successfully Deleted हो जाएंगे उसके बाद)
